Child with disabilities and Mental Illness

NGF provides respectful and compassionate care to individuals with physical disabilities and mentally ill child. Support for people with special needs and their families, including those with cerebral palsy, developmental disabilities, mental retardation, autism, and other disabilities. Services include information and referral, supported housing, case management, academic assistance service planning, support for activities of daily living, social skills groups, and individual advocacy.

Services Include

  1. Information, Referral, and Phone Consultation : The Disabilities Resource program is a free information, referral, and consultation service. The trust works with families, professionals, and people with a wide range of disabilities, helping them connect with programs and services that promote the fullest participation in the community. Callers receive professional assistance in identifying resources related to education, vocational training, housing, finances, religious life, recreation, and other aspects of community living. The trust helps callers navigate the various systems of support available to an individual with disabilities, and offers compassionate guidance that allows callers to consider steps for transition and futures planning.
  2. Care Coordination and Coaching : Our Care Coordination and Coaching staffs have experience working with Childs, adults with disabilities and mental illness, each of whom has distinct interests, abilities, as well as needs. We partner with each individual and his/her family to identify goals that will lead to a more meaningful and purposeful life.Whether it is expanding one’s social circle or coordinating a series of important medical appointments, our staffs support our clients to pursue personal goals while also attending to their overall well-being. Each service program is unique, and our services can be provided in the home, out in the community, or at our offices.We provide a range of individualized care coordination and coaching for individuals with disabilities and mental illness including, but not limited to:
    • Healthcare coordination and medication management
    • Access to social and recreational activities
    • Financial literacy and ongoing assistance with money management
    • Household and personal organization
    • Health Lifestyle Coaching: improving healthy eating habits and physical activity
    • Advocacy and service plan coordination
  3. Specialized Services Coaching for individuals for cerebral palsy disorders Our Care Coordination and Coaching staffs are experienced clinicians who work directly with adults with Autism Spectrum Disorder, Asperger Syndrome, or related conditions. We collaborate with each participant and his/her family to achieve the participant's goals, leading to a more meaningful and purposeful life. We provide Individual and Family Supports including:
    • Coaching and Life Skills Training: Improving daily living, organizational, social, and vocational skills.
    • Care Coordination: Accessing benefits including health care, financial supports, housing opportunities, and medical care.
    • Family Consultation: Assistance with mid- and long-range planning including housing, ongoing support services, and financial needs.
    • Resource Navigation: Information and guidance in finding appropriate services and opportunities in the community.
